Why I do this cleanse every few years
I first came across this protocol in acupuncture school. I was skeptical — the idea of blasting gallstones out by drinking olive oil and grapefruit juice at 10 PM sounded extreme. But after reading Dr. Hulda Clark's book The Cure for All Disease and studying the liver's central role in metabolic health, I decided to try it myself.
The next day after doing this cleanse, I noticed 3 main things--3 things that nothing else could even touch. 1. Chronic tension in my shoulders was gone. 2. My allergies were not as bad. 3. I sprung out of bed in the morning like a spring chicken. The last one only lasted a few days, but it still showed me that something had shifted in my body.
That was over 10 years ago. I still feel this cleanse is one of the most impactful thing we can do for our health, and its so simple.
The liver processes many things: hormones, environmental compounds, chemicals and medications, emotions, metabolic waste, alcohol. It produces roughly a liter of bile per day, and that bile is your body's main route for clearing fat-soluble waste.
The raw material the liver uses to make bile is cholesterol-- so if the liver function is obstructed, cholesterol levels will be high. When bile flow slows down, everything downstream feels it. Digestion stalls. Energy dips. Your body's detoxification capacity drops. Lymphatic congestion stagnates all circulation, including pressure in the head and sinuses.
This protocol works by stimulating a large release of bile all at once. The magnesium sulfate relaxes the bile ducts so they can enlarge, and the olive oil and grapefruit juice trigger a strong gallbladder contraction, flushing out any sediment sitting in the gallbladder or liver ducts. This sediment might be old grease, tiny sandlike granules of minerals or bacteria clusters, or the beginnings of calcified gallstones.

Why practitioners pay attention to liver function
Your liver runs over 500 metabolic processes. Two of the most relevant to long-term wellness:
Phase I and Phase II detoxification. Phase I converts fat-soluble toxins into intermediate compounds. Phase II conjugates those intermediates with glutathione, sulfate, or amino acids so they can be excreted. When Phase I outpaces Phase II (common with high toxic load or poor nutrition), reactive intermediates accumulate. This is where oxidative stress starts.
Bile production and flow. Bile emulsifies fats, carries waste out of the liver, and delivers fat-soluble vitamins to the gut for absorption. Sluggish bile flow means poor fat digestion, reduced waste clearance, and potential buildup of cholesterol-based deposits in the gallbladder.
NAC (N-acetyl cysteine) is the body's primary precursor to glutathione, the molecule that powers Phase II detoxification. Before you begin: Read the entire protocol. Schedule 2 days where you can rest — start on a day that allows you to take the following day easy. Do not attempt this protocol if you haven't reviewed the safety section at the end of this article.
The 2-day protocol: step by step
Day 1: Preparation and the flush
8:00 AM — Zero-fat breakfast and lunch
Eat only zero-fat foods: cooked cereal (oatmeal is ideal), fruit, fruit juice, bread with preserves or honey. For lunch, plain steamed vegetables with white rice. No dairy, butter, oils, meat, eggs, nuts, seeds, or sweets. No protein fats of any kind. Eating fat could make you feel ill during the evening doses. The zero-fat diet allows bile to build up and develop pressure in the liver. Higher pressure pushes out more waste during the flush.
2:00 PM — Stop eating
Do not eat or drink after 2 PM. This matters. Eating later can cause nausea during the evening doses.
Make sure your Epsom salt doses are prepared. If using capsules, count out four groups of 15 capsules. If using crystals, mix 4 tablespoons Epsom salt + 1/2 teaspoon vitamin C in 3 cups water. Refrigerate. This makes four servings of 3/4 cup each.
6:00 PM — Dose 1 (Epsom Salt)
Take 15 capsules of magnesium sulfate with no more than 3/4 cup water. If drinking the crystal solution, take 3/4 cup. You can bite into a lemon wedge or rinse your mouth to cut the taste. Spit, don't swallow extra water.
Get your olive oil and grapefruit to room temperature now if you haven't already.
8:00 PM — Dose 2 (Epsom Salt)
Repeat: 15 capsules or 3/4 cup of the Epsom salt solution. You haven't eaten since 2 PM, but you shouldn't feel hungry at this point.
Finish all your bedtime chores now. Timing from here on is critical.
9:45 PM — Prepare the olive oil and grapefruit drink
- Pour 1/2 cup olive oil into the pint jar.
- Squeeze your grapefruit (washed twice in hot water, dried) by hand into a measuring cup. Remove pulp with a fork. You need at least 1/2 cup juice. You can add lemon juice to reach 1/2 cup. Pour the juice into the jar with the olive oil.
- Add 1/4 to 1/2 teaspoon citric acid. Close the lid and shake hard until the mixture turns watery. Only fresh grapefruit juice creates this emulsion.
- Set out 2 capsules of black walnut hull (or 1 teaspoon tincture) and 4 to 8 capsules of ornithine.
- Visit the bathroom. Even if it makes you late, go now. But don't be more than 15 minutes late for the 10 PM drink — timing affects how well this works.
Have these ready at your bedside: the grapefruit-olive oil drink, honey (optional), ornithine capsules, black walnut hull capsules or tincture, cinnamon (optional).
10:00 PM — Drink the olive oil and grapefruit mixture
Stand or sit upright. Get it down within 5 minutes (15 minutes maximum). A large straw helps. You can use cinnamon or honey between sips — have them pre-measured so you're not fumbling around.
Take the black walnut hull (2 capsules or 1 tsp tincture) and 4-8 ornithine capsules with your first sips. The ornithine helps you sleep through the night. Take 8 capsules if you tend toward insomnia.
Immediately lie down flat on your back, head elevated on a pillow. Do not get out of bed. Do not clean the kitchen. Do not check your phone. Stay perfectly still for at least 20 minutes. You may feel movement along your bile ducts — that's not painful because the Epsom salt has relaxed the duct valves.
Go to sleep. The sooner you're still and asleep, the better the results.
Expect loose stools in the morning. This is normal and expected.
Day 2: Completion and recovery
6:00 AM or later — Dose 3 (Epsom Salt)
When you wake up, take 15 capsules of magnesium sulfate or 3/4 cup of the Epsom salt solution with no more than 3/4 cup water. Do not take this before 6 AM. If you feel nauseous or have indigestion, wait until it passes before taking the dose.
You may go back to bed.
2 hours after Dose 3 — Dose 4 (final Epsom Salt)
Take the last 15 capsules or 3/4 cup of Epsom salt solution. You may go back to bed.
2 hours after Dose 4 — Begin eating again
Start with fruit juice. You can add another 1/2 teaspoon of citric acid to support continued clearance. Half an hour later, eat some fruit. One hour after that, you can eat a regular meal, but keep it light. Vegetables first. Avoid sweets and heavy foods.
By dinner time, you should feel recovered.

Practitioner notes from 10+ years
After doing this flush annually for over a decade, here's what I've learned:
Preparation matters more than the flush itself. The zero-fat day isn't optional. Every time I've heard of someone having a rough experience, they either ate fat too late or didn't fast long enough. The bile pressure buildup is the whole mechanism. Don't shortcut it.
The ornithine is not optional either. I skipped it my first time and was up half the night. Your body is doing real work during the flush. Ornithine helps the urea cycle and lets you actually sleep through it. Take the full 8 capsules if you have any history of sleep difficulty.
Your first flush is usually the most dramatic. After the first one, each subsequent year feels milder. I take that as a sign that annual maintenance keeps things moving.
Rest the next day. Plan nothing for Day 2. Your body just moved a significant volume of bile, and the magnesium sulfate acts as an osmotic laxative. Hydrate well and take it easy. I usually spend the day reading and drinking herbal tea.
This is not a substitute for daily liver support. A 2-day flush once a year does not compensate for 363 days of poor nutrition, chronic stress, or environmental toxin exposure. The flush is a reset — your daily habits are what maintain the results. More on that in the next section.

What actually keeps your liver healthy year-round
Periodic cleanses get the attention, but what you do the other 363 days matters more. Here's what I build into my own routine and recommend to clients:
NAC (N-acetyl cysteine) is the rate-limiting precursor to glutathione, your liver's primary detoxification molecule. Without enough glutathione, Phase II detoxification stalls and partially processed toxins accumulate. Cruciferous vegetables (broccoli, kale, Brussels sprouts, cabbage) contain sulforaphane and indole-3-carbinol, both of which upregulate Phase II detox enzymes. Raw or lightly steamed preserves the most sulforaphane. Heavy cooking breaks it down.

This cleanse involves a significant laxative effect from the magnesium sulfate. Stay near a bathroom for Day 2. Hydrate well throughout.
